Again, include copies (not originals) of documents that support your position. Many businesses specify an address for disputes. If the business reports the item to a credit bureau, it must include a notice of your dispute.

Most of my credit issues are resolved. However, I'm curious as to your next step once you resolved the medical bill situation.  Did you pay the creditor and subsequently write a letter to the credit bureaus? I have a $284 medical bill I can't recall not paying, but I would like to resolve the matter this year. 
The way a secure credit card works is simply. You prepay your credit ahead of time to “secure” all of your purchases. Your credit limit for this type of card is exactly the amount of money that you prepay. This basically makes you a zero risk debtor to the bank if you happen to not pay your bill. You can obtain a secured credit card for as little as $200, so there really isn’t any reason to NOT get one.
If you don’t pay a medical bill or a cell phone bill, your account may be referred to a collection agency. Once it is with an agency, they can register that debt with the credit bureau, which can have a big negative impact on your score. Most negative information will stay on your credit bureau for 7 years. Positive information will stay on your credit bureau forever, so long as you keep the account open. If you close an account with positive information, then it will typically stay on your report for about 10 years, until that account completely disappears from your credit bureau and score. If you don’t use your credit card (and therefore no payment is due), your score will not improve. You have to use credit in order to get a good score.

Of the major credit repair organizations, only Lexington Law has received an A rating from the Better Business Bureau. The Credit People and CreditRepair.com received high ratings from their consumers online, but are not rated by the Better Business Bureau. These companies don’t do anything you can’t do yourself, but they may be worth your money if you’ve got a lot of negative information to remove.

What those consumer credit counseling businesses won't tell you (but they know all-too-well!) is that their services often do more harm than good! It's true! When they make so-called "special arrangements" with your creditors, all they are doing is making an agreement on your behalf that you will pay off a percentage of the debt! But the bad part is that your credit is still damaged!
Develop the good financial habits of living within your means, setting aside money in your emergency fund, and saving for the future. That way, you’ll be less inclined to skip payments, and you’ll have something to fall back on if you run into financial trouble. Keep with the good habits you formed while rebuilding your credit, and it will be easier to maintain your new, better credit history.
Despite what some advertisements may imply, there is no magic formula for rebuilding your credit score quickly. This is a marathon, not a sprint, so it may take a long time to reach your destination. However, if you take the right steps and avoid further mistakes, it shouldn't take too long before you can start to see progress in your credit score. That progress should encourage the positive habits that will eventually fully repair your credit.
